Document Required

1. A photocopy of the Birth Certificate of the student issued by the government authorities should be attached with this form.
2. The date of birth should be entered with particular care as no change will be allowed later.
3. The Registration form will be rejected if the information given is incomplete or found to be not true or if the required documents are missing.
4. This Registration form should necessarily be filled only by the parent of the child seeking admission (Guardian, on request).
5. All the entries are to be made carefully as any request for change will NOT be entertained.
6. Under no circumstances will the Registration form be accepted after the due date and time.
7. The Registration stands automatically cancelled if the child/parents are not present on the informed date(s).
8.This Registration from is NOT transferable to another year or child, once submitted.
9. No fees will be refunded if cancellation is made after admission..
10. In case of death of any one parent copy of death certificate is mandatory.

In the event of suppression or distortion of any fact made in this registration form, I understand that the decision of the school in this regard shall be final and binding upon me and I shall abide by the same.
I have fully understood the rules and regulations as well as the importance of a holistic approach to education, and am fully aware of my responsibility and contribution towards this and request that my child be registered. I undertake to abide by all the rules and regulations of the school in matters of admission and fees from time to time. In case of need to withdraw my ward, I understand that a refund of the fees cannot be claimed and agree to abide by this. I do accept that the admission is at the sole discretion of the Principal.
