Dear Parents, 

As per the DOE Circular No. DE.23(386)/Sch. Br./SHP/2026/45-51 for strengthening the implementation of HPV Vaccination Drive among eligible adolescent girls, kindly note that:

• Eligible girls are those aged 14 to 15 years (with a 90 days grace period on the day of vaccination)
• Cervical Cancer is a significant health problem in our country, which is also the only cancer preventable by a vaccine against Human Papilloma Virus (HPV). This HPV vaccination done at an early age in girls provide significant preventive protection against Cervical Cancer.
• This campaign has been launched with the aim to provide one dose of HPV vaccine to all eligible girls of age 14 to 15 years (Free of Cost).
• The vaccination is being conducted at Government Health Facilities by a trained vaccination team (Ayushman Arogya Mandir, Babarpur) and is being digitally recorded on U-Win portal.

The format of the consent form and the HPV awareness video link are attached herewith. Kindly take a printout of the form, fill it properly and submit it to the class teacher by 20^th May, 2026.

1. Kindly don't bring your vehicles near the school gates to avoid unwanted rush at the gate.
2. Kindly come at the arrival and dispersal time allotted for the particular class for your convenience and bring Parents Id card whenever you come to school.
3. It is mandatory for you to give details to the class teacher about the person authorized by you to pick and drop your ward. Do not change the authorized person frequently.
4. All the details about your wards' siblings studying in the same school, contact number, address, medical condition (if any), etc. should be intimated to the class teacher.
5. Kindly advise your ward to stay within the School campus and remain with the Class Teacher in case you or rickshaw / van drivers are not there to collect them timely.
6. We solicit your cooperation for the smooth and safe dispersal of your ward from the School Campus.

You are again requested not to block the School Gate at the time of Arrival & Dispersal.
