General Instructions:

The age of the child should be 3+ as on 1st April 2023.

All fields marked with * are mandatory.

Scanned copy of following certificates to be uploaded:

Birth Certificate of the Child
- Name of the Child must be printed on the birth certificate
- Name of the parents must be same as that mentioned on the birth certificate

Photograph of the child

Address Proof
Any one of the following will be considered as Address Proof:
- Aadhaar Card of Father/Mother
- Ration Card with names of parents mentioned
- Voter I card
- Electricity Bill/MTNL Telephone Bill/Water Bill/Passport in the name of either parent
- Rent agreement signed in the last six months

ID Proof of the parents/legal guardian
*Please note that only Aadhaar Card/Voter I Card/Pan Card will be considered as valid identity proof of the parents/legal guardian
*File size should not be more than 1 MB and only JPEG, GIF, PNG, PDF formats are allowed.
- Original Documents have to be produced at the time of admission.
- If you are applying for more than one child, please use a different email id for the second child
- Provision of email for communication is important.
