Birth Certificate issued by Municipal Corporation or
issued by Govt. Hospital shall be accepted.
Please upload the latest passport size, coloured
photographs of the student, parents or guardian (if
applicable).
Original Transfer Certificate from school last attended
must be submitted within one month of admission.
Incomplete or wrongly filled forms will be rejected.
Submission of this registration form does not guarantee
admission which will depend on the number of vacancies.
The duly filled form should be submitted in school
office within a week.
Parents are advised to inform school authorities with
relevant authorised document in case of any change of
information regarding the ward.
