General Instruction for Admission :

Step 1 / 6

Filling up the entire form is mandatory. Incomplete, illegible or over written Registration Forms will not be considered for admissions. In case the registration form is filled by guardians other than parents, it should be clearly indicated. The Registration Form should reach the school latest by . Registration Forms submitted after the last date will not be considered for admission. The following documents must be attached with this form:

Latest colored passport size photo graph of the student (One duly pasted on the form and two enclosed along).

Latest colored passport size photo graph of the parents (Father&other).

Attested photocopy of student’s birth certificate issued by a competent Village/Municipal/Government Authority (Affidavits or school certificates are not acceptable to certify DOB). Original Birth Certificate for Verification.

Attested copy of the report card or mark sheets of the Last Yearly/Half Yearly examination of the previous class and school.

Address Proof: Pan Card/ Driving License/ Electricity Bill/ Adhaar Card/ Passport of parents.

SC/ST/OBC/Others certificate (if any).

Undertaking duly singed by the parents.

Pupil’s Medical Check-Up Report duly filled in by a doctor.
