INSTRUCTION

Please attach along with:
• A self attested copy of the last Report Card of the previous school if applicable.
• A self attested copy of the Birth Certificate issued by the Municipal Corporation.
• An affidavit (original) of caste proof in case of SC/ST/OBC category.

GUIDELINES

• Parents must read the school prospectus in detail.
• Swaraj India Public School is a co-educational English medium school from classes I to XII.
• The registration form must be filled in neatly and correctly and submitted in the school office well in time as per instructions given. Further admission procedure will be as instructed by the admission office.
• Minimum age for admission as on 1 April 2020 is:
  Class - I      5 years and 6 months
  and a difference of one year for each subsequent class.
  
• Parents are required to submit a self attested copy of the original Birth Certificate of their ward issued by the Municipal Corporation along with this form. Original Birth Certificate must be shown at the time of interaction.
• Once admission is granted (in class II and above) original Transfer Certificate must be submitted. In case a student seeking admission is coming from a school of another district the Transfer Certificate (T.C.) should be counter signed by the District Education Officer or the Inspector of School of that district.
• Admission will be granted provisionally subject to the submission of original Transfer Certificate from the previous school. Admission will be considered cancelled if original Transfer Certificate (T.C.) is not produced within one month of admission.
• This registration form is valid only for the session in which it has been obtained.
• An interaction and a qualifying entrance examination will be held class I upwards in the subjects of English Language, Math and Science.
• Fee once deposited is not refundable/transferable under any circumstances.
• Date of birth cannot be altered once admitted.

DECLARATION

Acceptance of this Registration Form in no way guarantees admission as admission is purely on the availability of seats and fulfilment of the criterion for admission as laid down by the school. The decision of the institution Head in this regard will be final.

I understand that admission fee once deposited cannot be refunded or transferred.
