The date of birth given in this forms as per the entry in the birth certificate and my child's transfer certificate from the school last attended (if applicable). I fully understand that the school will not entertain any subsequent request or change in date of birth unless supported by an affidavit from the court and an original newspaper release of the name change.

I agree to my child's photograph appearingn any Birla Open Minds brochures or publications or any mass media. The undersigned certifies that the following are acceptable to him/her-

1. Fee once paid are neither refundable nor transferable under any circumstances.
2. Management's right to implement changes in the fee structure and curriculum from time to time as the circumstance may warrant.
3. A minimum of 75% attendance is compulsory for promoting the child to the next higher class, along with other criteria.
4. While Birla Open Minds, shall take utmost care of children when they are in school custody and ensure that it shall comply with all possible safety measures to prevent any accident or mishap, I agree not to hold the school responsible for any possible illness, accident or injury during classes, school organized activities or on the school premises. I hereby verify that I have read, understand and accepted the statements above.

The undersigned certifies that each part of the application and the information inserted here in has been carefully read and is found to be true and correct. I undertake to abide by the terms and conditions.

I / We fully understand that the school, on accepting the application of my/our ward, is not in any way bound to grant admission as the admissions are granted purely on the availability of seats & on qualifying the pre-admission interaction. I / We agree that the decision of the Principal regarding admission will be final and binding. I / We agree to meet all the financial responsibilities with respect to the admission of my /our ward to the school.

I / We certify that my/our ward will satisfy the eligibility requirement for admission to the class applied for, as stated in the application material of the school. I / We understand that any false/incorrect information given by me/us shall render this application invalid, and consequently, the admission granted based on it is liable to be cancelled by the school.

In the event of any injury or harm or loss of life during the course of the stay of my/our ward in the school, I/we shall not hold the school or authorities responsible for the same. I / we understand that three months' notice needs to be given to the school in case I / we wish to withdraw my/our ward. I / We understand that in such case only the security deposit is refundable.

During the year, all of our students discover, learn and achieve many academic and developmental goals. We regularly take photographs and videos of enrolled students to be shared with other social media attending in a very safe manner. Please note that the usage of photos or videos by enrolled families of any child other than their own is not allowed.

Birla Open Minds International School may also use students' photos, videos, and reels in communication with the outside community for information on school activities or promotions in different media formats.

In consideration of my child(ren)'s participation at Birla Open Minds International School, I hereby grant permission to the School, its staff and affiliates to utilize my child(ren)'s photos, videos or artwork in communication with the outside community for the above-mentioned purposes. I understand that no royalty, fee or any other compensation of any kind shall become payable to me by reason of such release and use of any photograph or video till my child is in school and even after he/she leaves the school.
