1. Attested photocopy of Municipal Birth Certificate (original) to be produced at the time of submission of form /admission.

3. One passport size photograph of mother and father each.
4. Medical Fitness Certificate.
5. Proof of residence-attested photocopy of Ration card/Voters Identity Card/ Smart Card /Passport / Electricity Bill.
6. Certificate of the category(SC/ST/OBC).
7. Transfer Certificate.
